It will depend on your income and circumstances. (It seems strange to me, but people seem more worried about fees than about practical pursuit of mental health.)

What You Want From Therapy
Comment in the space below...what caused you to seek therapy...what do you want from engaging in therapy...be as specific and detailed as necessary to convey whatever issues or problems you are facing. Let it all hang out!...

 

About Your Physical Health And Psychological History
What matters here is whether you have a medical condition that would be significant regarding your moods, emotions, attitude or outlook. If so, tell me about it.

  • Are you taking any prescription medications?
  • Have you ever been in therapy? When? For how long? Was it effective?
 

A Bit About Your Past
Were you a happy kid? Did you have friends?

Tell me something about what it was like growing up. Describe your parents. Do you have brothers and sisters? Are you in touch with any members of your family? Do you get along with them?

 

Something About Your Present Situation
Are you involved in a current relationship? Are you happy with your job? Are you going to school? What are your hobbies, leisure activities, goals?

 

And...Some Important Issues
If the answers to these questions were not important, I wouldn't ask them. However, I consider these areas highly significant to the therapy process, so I always explore them during a first interview.

  • Are you suicidal?
  • Are you homicidal?
  • Do you use alcohol or recreational drugs?
  • What is your sexual preference? Heterosexual? Homosexual? Bisexual?
  • Were you abused as a child?
  • Are you in an abusive relationship now? If so, who is doing the abusing?
